Durham Connects is a program of The Center for Child & Family Health, as part of the Durham Family Initiative, in partnership with the Center for Child and Family Policy at Duke University, Durham County Social Services, and the Durham County Health Department. It is funded through the Duke Endowment, Durham County, local grants, and partnerships.

Providing in-home nurse visits to every parent in Durham County is a large endeavor, and many groups have contributed to its launch and success.
