Durham Connects is a partnership of the Durham County Health Department and the Durham Family Initiative. It is funded through the Duke Endowment.

Providing in-home nurse visits to every parent in Durham County is a large endeavor, and many groups have contributed to its launch and success. Among them are:

• Durham’s Partnership for Children, a Smart Start initiative
• Network of Care

• focus groups, pilot programs and advisory members consulted during the planning stages.
