Referrals or "connections" to agencies is at the core of Durham Connects' success. Durham Connects has created an exhaustive list of agencies serving residents with children 0-5 years of age. Durham Connects’ nurses are experts in Durham's parent resources and make the best fit according to families' needs and desires.

Durham Connects follows up with parents after their home visit to ensure that families have everything they need. Durham Connects respects patients' privacy and complies with all HIPAA laws. Families may sign a release of information allowing the nurse to correspond with referral agencies and pediatricians.

Durham Connects regularly communicates with agencies to ensure services are available and not over crowded. Agencies are encouraged to communicate regularly with Durham Connects staff to discuss appropriateness of referrals, capacity issues or other areas of concern.

Durham Connects collects referral data to identify trends in new parent demand. This information may be useful to agencies in justifying increases in funding or reallocation of existing funds. In some cases Durham Connects may identify gaps in service, and the Durham Connects team works to fill these gaps.
